By checking, I acknowlegde that in addition to the above, St. Mary's needs a copy of the child's Birth Certificate and a Letter of Good Standing for parents and/or for the godparents, if they are not parishioners of St. Mary's. A Baptismal preparation class is also required for parents and godparents, unless there is record of taking one in the past two years. 

By checking, I acknowledge that for a person to be a godparent, they must be a fully initiated Catholic (received Baptism, Eucharist, and Confirmation) that is in good standing with their Catholic Parish.
